been mining for hours, had 22 diamond, around 70 gold and 40 lapis lazuli, and I saw some coal. For some stupid reason, I dug straight down into the coal, and fell straight into lava. the lava was deep, and while rising to the surface, my player got stck underground in the lava, and no items were saved, including the full set of diamond tools and armor I had with me.
Also, first time playing minecraft, and I had just survived my first night. I dug out of my tiny cave, and looked around during my 2nd day. I had enough time to say to myself, "What is that moving green tall thin"..BOOOOOM. I rage quit for a few days, and have been veeeeeery cautios with creepers from then on.

not realy a fail, but funny
on a server, i made a rail system from my basement (at bedrock level) to my actual house (at ocean level) and one day i was to lazy to use the rail system (and the cart was gone) so i climbed down the ladder, and when i got down there, i saw a creeper in the minecart, it blew up, destroyed a few rails, and almost made lava flood the basement
On my first world, I survived my first night and made a mine. When I was mining I heard the creepy sound thing that you hear if you're near a dungeon. I scared the **** out of me and fell in a cave. I was stuck here for weeks.
Well, Now that I remember a fail, here's how it went.
I was creating a Tree-house atop this one medium-sized hill that I made my home in, and as I'm building the frame out of wood, I fell, plummeted about 40 blocks, landed on a Ninja Creeper, and it blew up, sending me up about 5 blocks, making me land at my doorstep.

I'd have to say...uh....I was on a SMP server and this guy was handing out loads of TNT.We were doing a war,but when I was going to sabotage their base with TNT,I accidentally put them on mine xD InstaBAN!
