To make a custom profile picture, you will need to draw an image on either DSiPaint or 3DSPaint. Once you have drawn an image and saved it, navigate to your user settings by clicking on your avatar in the top right corner of the page and clicking on Settings under Maintenance. Scroll down until you see the image you drew under "Site Avatar".
